This notice explains how the practice collects, uses and protects your personal information under the Protection of Personal Information Act 4 of 2013 (POPIA).

2. What we collect
Your name, contact details, date of birth, ID number and medical aid details; your health information (case history, consultation notes and any remedies dispensed); your booking history; messages you send us; and basic website usage information collected through cookies.
3. Why we use it
To provide consultations and dispense remedies; to manage bookings and send appointment reminders; to issue invoices and support medical aid claims; to keep the clinical records the law and our professional rules require; and, only if you agree, to send you practice notices. You can opt out of notices at any time by replying STOP.
4. Your health information
Health information is special personal information. We process it only as POPIA allows for health care providers bound by professional confidentiality, or with your consent. We never sell or rent your personal information.
5. Who we share it with
Only where needed: your medical aid scheme if you ask us to support a claim; our practice-management and booking software providers, who must keep it confidential; a health professional who referred you or to whom we refer you, with your consent; a registered colleague who covers the practice while we are closed; or where the law requires it.
6. How we protect it
Records are kept in access-restricted, password-protected systems. If we suspect your information has been accessed without authority, we will tell you and the Information Regulator as POPIA requires.
7. How long we keep it
For as long as the law and our professional rules require, after which it is securely destroyed.
8. Your rights
You may ask to see the information we hold about you, ask us to correct or delete it where the law allows, object to how we use it, and withdraw consent you have given. Email info@drmacintosh.co.za. We may need to confirm your identity first.
